For a company selling into physician offices: which practices in a geography actually see the patients you care about. Built from CMS public-use files and the NPI registry, joined on the NPI. It turns "a practice exists here" into "a practice that treats your indication exists here", and says who runs it.

• Medicare fee-for-service only. A practice appears only if it billed Medicare for eleven or more beneficiaries. Cash-only, concierge, pediatric and commercially insured practices are absent from the ranked list, and the second list below exists to show them as present but unmeasured.
• About two years old. CMS publishes these files annually with a lag. Good for what kind of practice this is; wrong for what it did this quarter.
• Blanks are suppression, not zero. CMS withholds small cells to protect patients. A missing percentage means fewer than eleven patients, not none.
• The ranking is a judgement. Fit is the share of the practice's Medicare patients with the condition; scale is the log of how many patients it sees; prescribing adds a bounded boost. All three are shown per row so the ordering can be argued with.
• This profiles named practices from public record. Every field is published by CMS or the NPI registry. It is shown here to demonstrate the join, capped to the top results, and marked as a demonstration rather than offered as a prospect list.

This is a portfolio demonstration. The logic runs and the sources are live, but it was built to show an approach on a realistic problem, not deployed against a real one. Nothing here has been validated for clinical, commercial or investment decisions, and no result should be treated as advice.
